Трихомониаз - одно из самых распространенных заболеваний, передающихся половым путем. Раньше его считали достаточно безобидным и легко поддающимся лечению. Но в настоящее время установлено, что перенесенный трихомониаз может стать причиной бесплодия, а также осложнений во время родов у женщин и хронического простатита у мужчин. Значит, к этому к этому заболеванию стоит отнестись со всей серьезностью.
Что такое трихомониаз и как им можно заболеть
Трихомониаз - воспалительное заболевание мочеполовых органов человека,
передающееся половым путем. Вызывается трихомониаз влагалищными
трихомонадами - одноклеточными простейшими, которые способны
существовать не только внутри организма человека, но и вне него.
Питание трихомонад происходит путем поглощения клеток, в том числе
бактерий (например, гонококков), которые могут долго сохранять свою
жизнеспособность внутри трихомонад. Этим объясняется многочисленные
рецидивы (обострения заболевания) гонореи при сочетании ее с
трихомониазом. Трихомонада быстро погибает при температуре свыше 40º и
в дезинфицирующих растворах.
Трихомониаз очень распространенное заболевание, передается оно в
основном половым путем, но встречаются случаи передачи и бытовым путем
через предметы личной гигиены, например, в бане.
Как протекает трихомониаз
Инкубационный период (время от заражения до проявления признаков
заболевания), как правило, составляет от 5 до 15 дней. Различают острый
свежий трихомониаз, хронический (если заболевание длится более двух
месяцев) и бессимптомный (трихомонадоносительство).
Для трихомониаза у женщин характерно множественное поражение
мочеполовых органов (влагалища, мочеиспускательного канала, мочевого
пузыря и т.д.). При остром течении у больных трихомониазом женщин
появляются желтые, иногда с неприятным запахом выделения из влагалища,
зуд, покраснение и припухлость наружных половых органов, частые
болезненные мочеиспускания, боль при половых контактах.
Мужчины чаще страдают хроническим трихомониазом или являются носителями
трихомонад (и источником распространения инфекции), но встречаются и
острые проявления: выделения из мочеиспускательного канала, боль и
жжение при мочеиспускании, боли в промежности (если воспалилась
предстательная железа). Поражается мочеиспускательный канал,
предстательная железа, семенные пузырьки, яички.
Степень выраженности заболевания зависит от общего состояния организма,
возраста и сопутствующей инфекции. Так, при наличии гонореи и кандидоза
(грибковое заболевание - молочница) трихомониаз с трудом поддается
лечению и часто переходит в хроническое заболевание.
Осложнения трихомониаза
У женщин трихомониаз может быть причиной бесплодия и различной
патологии беременности, например, преждевременных родов и
преждевременного излития околоплодных вод.
Длительное бессимптомное течение трихомониаза у мужчин может приводить
к мужскому бесплодию за счет поражения продуцирующих сперматозоиды
органов. Кроме того, трихомонады, находящиеся в сперме, снижают
жизнеспособность сперматозоидов и обездвиживают их.
Диагностика трихомониаза
Для постановки диагноза у женщин исследуют выделения из влагалища, у
мужчин - выделения из мочеиспускательного канала и секрет
предстательной железы. Под микроскопом смотрят окрашенные специальным
способом мазки и выявляют возбудителя инфекции. Проводится также
микробиологическое исследование с посевом выделений на питательные
среды и последующим выявлением возбудителя.
В крови методом ПЦР можно обнаружить следы ДНК трихомонады - это более точный метод.
При хроническом течении и бессимптомном носительстве наличие трихомонад
в организме выявить достаточно трудно, для этого требуется
неоднократное комплексное обследование.
Лечение трихомониаза
Учитывая длительность течения, склонность к хроническому течению и
носительству, лечение трихомониаза должно проводиться только после
предварительного тщательного обследования и включать в себя как
противотрихомонадные препараты, так и общеукрепляющие средства.
Кроме того, назначается местное лечение для снятия местных
воспалительных явлений (ванночки, вливания в уретру и т.д.) и
физиотерапевтические процедуры.
Критерием излеченности трихомониаза считается наличие отрицательных
результатов анализов на 1-ый, 2-ой и 3-ий день менструального цикла в
течение трех месяцев.
Профилактика трихомониаза
Профилактика трихомониаза - это моногамная половая жизнь (с одним
партнером). Если же появилось подозрение на наличие трихомониаза, то
обследоваться и лечиться должны оба партнера, иначе возможно повторное
заражение.
Помните! Трихомониаз не так безобиден, как это обычно считают!
